HS to host COVID-19 vaccine clinics

We are partnering with the Broome County Health Department to offer vaccine clinics for high school students ages 16 and up. While parents do not need to be present, they must provide consent ahead of time. The first dose will be provided during school hours on May 13, followed by the second dose on June 3. Participants will receive the Pfizer-BioNTech vaccine for free.
Before consenting, parents and guardians should review the Face Sheet for Recipients and Caregivers: https://www.fda.gov/media/144414/download

Or, you can download a paper consent form and return it to the high school. To download a paper consent form, click here.
