Physical Forms
The following forms are required for private/sports physicals.
Private Physicals
The private physical forms are required for any student having a physical done by their own health care provider. Physicals are required for all 4th and 7th grade students, new entrants to the District and any student wishing to have their health care provider complete their sports physicals.
School sports physicals
This form must be completed and signed by the parent for any student wishing to have a sports physical at school. Physicals will be done again in November by Dr. Jones our school physician or with Dr. Driscoll at the High School on August 16, 2010 at 8:00 am.
